Another option to go there is take the bus "D", and go to the end of route and from there you have to across a few building and go to the road (more less 800m). You will be in the same place from first option but on the second side of road ;).
It's very good place, a lot of times I catch the ride from there directly to Warsaw :). If you go to Warsaw use the sign "Warszawa". Good luck.

Take one of the trams: "1", "7", "15", "21" - direction "POŚWIĘTNE", and go to the end of the route. Then go to the other side of the Żmigrodzka street and start hiking at the bus stop. You will be well seen by the drivers who stop at traffic lights.
You can also take the bus "129" - direction "OŚRODEK REHABILITACJI", get off at the "POŚWIĘTNE" stop and start hiking :)
