A quick way to leave to a new destination is to look at the city bus map, many bus lines extend very far out of the city (up to 40km..). Take a city bus that goes to a village that's on your way and you get out of the bus when you're out of the city and see a good spot to hitchhike.

Hitchhiking out

Getting out of Hanoi is pretty easy if you go by bus to the beginning of the Expressway. It's a good place to get a ride. Towards Saigon

There's a city bus going to Phu Ly, along the AH1 take it to get out of the city and stop anywhere you think is good to start hitchhiking. On the Hanoi - Saigon highway hitching with trucks is very easy.
